Punch cutter, Surgical Punch??

I've been considering getting a cigar punch. But from some of the threads about them, I think most are too small (too much build up of tar). So I've been trying to find a punch that would remove most of the bottom of a large ring cigar (48-54), I figure it should be 14mm-16mm. I've found razor sharp surgical punches at 12 mm at this site (minimum order is $35 with $11 bucks shipping). I'm still looking for something even larger. Here is a possibility but I don't know if it will be sharp enough. Any thoughs on sizes or where to get something like this?
