I guess that the simple fact is that nicotine is an effective vasoconstrictor, and whatever form, it interferes with working and can be the final straw under some circumstances. A guy with red man on the ball field is probably safer than a guy in arctic temps doing monotonous strenuous labor like shoveling. Millions of times a year people go out and shovel, and once in a while you find a clear cut connection, but I honestly believe that this risk is probably greatly overstated. Driving without a belt is very dangerous but snow shoveling is a little less clear.
